How to Access Your Medical Records
- Patients can request copies of their medical records by submitting a formal request to the Health Records Department.
- LACGH adheres to Ontario’s Personal Health Information Protection Act (PHIPA) to protect the privacy and confidentiality of personal health information.
- All requests for records are processed securely, with confidentiality ensured throughout.
