How to Apply for Long-Term Care

Admission to our Long-Term Care Home is coordinated through Home and Community Care Support Services South East.

Here’s how the process works:
  • Begin with a referral through Home and Community Care
  • Assessment of eligibility and care needs will be completed by a Care Coordinator
  • If approved, your name will be added to a waitlist for available placement
  • LACGH will contact you or your Care Coordinator when a bed becomes available

What to Expect Before Admission

Before moving in, we recommend reviewing the following:
  • Packing List (see Living Here: Daily Life & Practical Info)
  • Policies on visiting, valuables, and smoking
  • Any special dietary or medical considerations
  • Review and sign necessary admission forms provided by your Care Coordinator

 

Our team will schedule an orientation session to help you get familiar with our Home and staff.

During Your Stay

Every resident receives an individualized care plan based on their needs. During your stay, you’ll have access to:

Residents are encouraged to participate in the life of the Home and bring familiar items to personalize their space.

Support for Returning to the Community or Another LTC Home

When it’s time to plan a discharge:
  • Your Home and Community Care Coordinator will guide the transition process
  • Discharge may include moving to another long-term care home or returning home with support services
  • Our team will help coordinate transportation and follow-up care arrangements

 

This ensures a safe, informed, and respectful transition.
