Freedom of Information (FOI)
Hospitals in Ontario have been covered by FIPPA since January 1, 2012. This gives you the right to request access to recorded information held by LACGH, including records of personal information (from January 1, 2007 onward). Requests for personal health information remain governed by Personal Health Information Protection Act PHIPA.
How to Make an FOI Request
- Written requests must provide enough detail to identify the records sought.
- Include your contact information and a $5.00 application fee (cheque or money order; no cash).
- Additional costs for processing (e.g., photocopying) may apply. You will be notified in advance.
- Requests can be for general records or personal information (excluding health records).
