Convalescent Care

The Convalescent Care Program provides short-term, restorative care for individuals recovering from illness, injury, or surgery.

Physiotherapy & Occupational Therapy

Our on-site Physiotherapy and Occupational Therapy Department ensures each resident receives an individualized rehabilitation plan.

Upon admission, residents are assessed for safety, equipment needs, and therapy requirements. Group rehabilitation sessions are held in the gym six days per week for one hour each day.

  • Average length of stay is approximately 45 days, with a maximum of 90 days.

Recreational Activities

We offer a variety of programs to promote socialization, activity, and well-being across the physical, emotional, cognitive, social, and spiritual aspects of care. Activities include Bingo, cards, music, pet therapy, and friendly visiting.

Dining Experience

Residents enjoy freshly prepared, nutritious meals designed by a clinical dietitian to meet individual preferences and dietary needs. Menus run on a 21-day rotation and are posted outside the Dining Room.

Meal Times

    • Breakfast: 8:00 a.m.
    • Lunch: 12:00 p.m.
    • Dinner: 5:00 p.m.
    • Three nourishing snacks are offered daily, each with multiple options.

Special Diets Available

  • Regular, dental soft, minced, and pureed textures
  • Healthy heart, diabetic, high energy/high protein
  • Thickened fluids, nutritional supplements
  • Alternate dietary needs can often be accommodated upon request.

Smoking Policy

LACGH is a smoke-free facility and property. Smoking cessation aids may be ordered at the resident’s expense. Residents who choose to smoke must do so independently outside the facility. A smoking release form is required on admission.

Discharge Planning

Our interdisciplinary team works closely with residents and families to prepare for discharge. A care conference occurs within six weeks of admission to discuss plans and required supports. Ontario Health @ Home may be consulted to coordinate services for a safe transition home.

What to Bring

Clothing

  • Underclothes, comfortable clothing, non-slip shoes/slippers with closed heels, sleepwear
    Note: Families or residents are responsible for laundering.

Toiletries (Scent-Free)

  • Toothbrush, toothpaste, deodorant, comb/brush, shaving items, denture care products, shampoo, razors
  • Please label all personal belongings, including dentures, glasses, and hearing aids

Additional Costs

While there is no program fee (funded by the Ministry of Health and Long-Term Care), residents are responsible for:

  • Prescription and dispensing fees
  • Transportation to and from appointments
  • Optional foot care and hairdressing services (arranged through the ward clerk)

Safety & Security

Valuable

Residents are encouraged not to bring valuables. A safe is available in each room upon request.

Building Security

All exterior doors are locked for safety. Please use the hospital’s main entrance.

Emergency Preparedness

A generator maintains critical operations during power outages. Fire drills occur monthly, and fire exits are clearly marked. The home does not allow extension cords or personal appliances such as fridges or toasters in rooms.

Air Conditioning

The facility is fully air-conditioned, with the temperature monitored by maintenance staff.

Resident Safety & Rights

Minimal Use of Restraints

Safety devices such as posture aids or seat belts are only used after assessment and with consent. LACGH maintains a least-restraint policy.

Abuse Policy & Mandatory Reporting

LACGH promotes zero tolerance for abuse. All allegations are investigated and reported as required by the Ministry of Health and Long-Term Care.

Whistle-blowing Protection

Residents, families, and staff are protected under the law for reporting concerns to the Ministry.

Feedback & Complaints

LACGH values your feedback. Residents and families are encouraged to share concerns or suggestions with staff at any time. If an issue is unresolved, contact the Director of Care or complete a formal complaint form available on the Unit’s communication board. All written complaints are reviewed by the Administrator or designee for follow-up.

Licensing & Quality Improvement

The Convalescent Care Unit is fully licensed by the Ministry of Health and Long-Term Care. The unit’s Quality Improvement Plan (QIP) is submitted annually to Ontario Health Quality as part of the hospital’s corporate QIP.
