If you wish to access or obtain copies of your personal health information, or request amendments, please follow these steps:
We will respond to your request within 30 days, unless an extension is required under law.

Additional costs (e.g. photocopying/research) may apply; you will be notified in advance.
If you wish to withdraw consent or opt out of certain disclosures such as the Patient Directory, or disclosure of religious affiliation, please complete the “Withdrawal of Consent/Opt-Out Form” and submit it to the Privacy Office.
We may share your PHI with other health service providers, provincial health systems (e.g., eHealth Ontario), community care access centres, and third-party vendors who assist in delivering health services to patients. All external disclosures are subject to appropriate legal, contractual and security safeguards.
If you believe your privacy has been compromised or your rights under PHIPA or FIPPA have not been respected, please contact the Privacy Officer immediately. You may also make a complaint directly to the IPC:

We will investigate your complaint and take corrective action where necessary. Remediation may include policy updates, additional training or changes to systems and practices.
