Basic Function and Scope:

The pharmacist will provide individualized, patient-centred pharmacy care as part of a multi-disciplinary team for inpatient and outpatient clinical services, including Oncology. Under the direction of the Director of Pharmacy, the pharmacist will participate in all aspects of medication management, including drug use management, drug distribution, education, training, and drug information. Provide leadership and support for staff involved in the medication use process according to departmental policies, provincial guidelines and Professional Standards of Practice.

Pay Rate:
$50.17-$66.63

Qualifications & Requirements

  • Bachelor of Science in Pharmacy/Doctor of Pharmacy (PharmD) from an accredited university OR an equivalent combination of training and experience as determined by the Ontario College of Pharmacists,
  • Current Part A Pharmacist registration with the Ontario College of Pharmacists.
  • Accredited Hospital Pharmacy Residency or at least 2 years of previous Hospital Pharmacy experience preferred.
  • Experience in direct patient care, drug information, pharmacokinetics and computerized drug order entry.
  • Strong knowledge of drugs and therapeutics with the ability to apply in the provision of patient care.
  • Demonstrated proficiency as a hospital pharmacist based on CSHP and OCP Standards of Practice and in NAPRA Professional Competencies.
  • Demonstrated ability to make sound professional decisions in a complex interprofessional environment.
  • Excellent interpersonal skills with demonstrated ability to communicate effectively, courteously, and tactfully (written and verbal).
  • Excellent critical thinking, problem-solving and autonomous decision-making skills.
  • Demonstrated ability to take initiative, work independently and as a team member in a fast-paced, challenging environment.
  • High degree of accuracy and attention to detail.
  • Demonstrated computer literacy, including experience with MSOffice software
  • Proven ability to attend work regularly.
