Basic Function and Scope:

The RAI RPN is responsible for completing and coordinating comprehensive resident assessments using the InterRAI LTCF tool in compliance with Ontario Regulation 246/22 under the Fixing Long-Term Care Act, 2021. This role supports person-centered care planning, quality improvement, and data accuracy by ensuring timely and accurate assessment submissions. The RAI RPN works collaboratively with the interdisciplinary team to ensure resident needs, preferences, and health status are effectively captured and integrated into individualized care plans. This position also plays a key role in the admission and discharge planning process, ensuring smooth transitions and proper documentation.

 

Hours of Work:
Rotating shifts (days, evenings, nights, and weekends), as required
Pay Rate:
$38.48-$39.01

Qualifications & Requirements

 

  • Graduate of an approved Nursing Program;
  • C.L.S current certification.
  • Completion of RAI-MDS training, including InterRAI LTCF coding standards

 

  • A member in good standing with the College of Nurses of Ontario;
  • Membership in good standing with Registered Practical Nursing Association of Ontario (RPNAO), preferred.
  • 1–2 years of clinical nursing experience in long-term care or a similar healthcare environment preferred
  • Excellent customer service skills;
  • Demonstrated accuracy and attention to detail in clinical documentation
  • Proficiency with technology
  • Excellent interpersonal, communication, and collaboration skills
  • Ability to prioritize work and manage multiple tasks while meeting tight regulatory deadlines
  • Demonstrated knowledge of the CNO Standards applicable to the RPN scope of practice;
  • Ability to work in a fast paced, multifunctional work environment;
  • Demonstrated commitment to ongoing education;
  • Excellent organizational and interpersonal skills with demonstrated ability to communicate effectively, courteously and tactfully;
  • Must have previous proven good performance and attendance record

The applicant must be able to meet the physical demands of this position.
