We are seeking a Nurse Practitioner (NP) to join our clinical team. The NP will provide advanced nursing care, clinical expertise, and leadership in the management of residents’ health. Working collaboratively with the multidisciplinary team consisting of Physicians, Nurse Practitioner team, the Director of Care, Registered Nurses, and Personal Support Workers, the NP will support evidence-based care, improve resident outcomes, and strengthen capacity across the care team.
In addition to providing care within the Long-Term Care Home, the NP will have opportunities to gain valuable experience as part of the broader LACGH team, supporting Convalescent Care and other hospital departments as needed. This provides a unique opportunity to build skills in both long-term and acute care settings, ensuring continuity of care and professional growth.
